1st Quarter
· Watch Bruce Madsen DVD - 6pts
· Health & Wellness Check within 60 days of program start - 6pts
· Select a Team (County) Captain - 3pts
· Implement 10 minutes of group activity 3x per week - 6pts*
· Individual activity at home at least 10 minutes once per week - 6pts*
· Implement Wellness Council within the county - 6pts
2nd Quarter
· Monthly education on nutrition - 3pts
· Reduce unhealthy snacks and sodas - 1pt
· Increase group physical activity to 15 minutes 3x per week - 6pts*
· Increase individual physical activity at home to 15 minutes at 2x per week 6pts*
3rd Quarter
· Develop teams within county e.g. by office - creating competition within the county - could have each person give $10 to put into pot as well as county for winning team - 3pts
· Monthly training on hydration - 3pts
· Increase group physical activity to 20 minutes 3x per week - 6pts*
· Increase individual physical activity at home at least 20 minutes 3x per week - 6pts*
· Continue to reduce unhealthy snacks and sodas replacing with water and healthy alternatives -1pt
· Invite local media to do story on changing the county’s health - 3pts
4th Quarter
· Create community walk-a-thon or something similar and invite media - 6pts
· Increase group physical activity to 25 minutes 3x per week - 6pts*
· Increase individual activity at home to 25 minutes 4x per week - 6pts*
· Assemble folder of healthy recipes to create county cookbook - 6pts
· Monthly training on benefits of exercise - 3pts
· Exit weight check - 2pts
*For exercise activities, points will count double for field personnel in Road Districts and Sheriff’s offices that participate.
